PROOF OF RESIDENCY

  • Evidence of residing in the state of Pennsylvania for 24 months

Pipetting Samples and Test Tube

TB TEST

  • You can submit any of the following (test must be from within 12 months)

  1. QuantiFERON TB gold

  2. 2 Step PPD

What To Expect After Completion Of Your Application

A caresphere representative will review your application within 24 hours. Keep an eye on your inbox for an email detailing any missing items. Just respond to that email by attaching the necessary documents. Once all the required items are received, we will schedule your orientation session.
