COMMUNITY HEALTH CHOICE WAIVER
Community Health Choices (CHC) is Pennsylvania's mandatory managed care program for individuals who are eligible for medical assistance. CareSphere is contracted with the four Medicaid appointed managed care insurances: UPMC, PA Health & Wellness, AmeriHealth Caritas at Pennsylvania, and Keystone First.
MEDICAID ACT 150 WAIVER
If you have a physical disability, the state-funded Act 150 program may be available to you to continue to live in your home and community with support and services.


MEDICAID CONSOLIDATED WAIVER
The Pennsylvania Consolidated Waiver is designed to help individuals with an intellectual disability, autism or developmental disability to live more independently in their homes and communities and to provide a variety of services that promote community living, including self-directed service models and traditional, agency-based service models.
PHYSICAL HEALTCHOICES PROGRAM
The Physical HealthChoices program is the name of Pennsylvania's mandatory managed care programs for Medical Assistance recipients. Through physical health managed care organizations (MCOs), recipients receive quality medical care and timely access to all appropriate physical health services. CareSphere is contracted with the MCO's Highmark Wholcare, UPMC For You, United HealthCare, Geisinger Health Plan & AmeriHealth Caritas.


OPTIONS PROGRAM
Pennsylvania's Options Program provides aging state residents with care services in their homes, such as personal care, home health, and home delivered meals. Assistance is also provided to family caring for a loved one at home, such as adult day care, respite care, and specialized medical transportation assistance.
CareSphere is contracted with Lehigh County AAA, Monroe County AAA, Bucks County AAA & Northampton County AAA to provide services to approved members.
VETERANS AFFAIRS
Veterans of the United States Uniformed Services may be eligible for a broad range of programs and services provided by the Department of Veterans Affairs (VA). Eligibility for most VA benefits is based upon discharge from active military service.


TRUST FUNDS
If you have a trust fund, ask your Trust Fund adviser. You might be eligible to use that fund for home care needs
PRIVATE INSURANCE
Many private insurance companies offer home health care services as a benefit. We will check with your carrier to determine the benefits provided.


MEDICARE
Medicare is a federal health insurance program for people age 65 or older and certain disabled people under 65. It provides care and support services to individuals who have home health care needs for a limited duration.
COMMERCIAL INSURANCE
Your primary health insurance may be able to cover your home care or home health care needs. CareSphere accepts most of the commercial insurances.
