Many private insurance companies offer home health care services as a benefit. We will check with your carrier to determine the benefits provided.
COMMUNITY HEALTH CHOICE WAIVER
Community Health Choices (CHC) is Pennsylvania's mandatory managed care program for individuals who are eligible for medical assistance. CareSphere is contracted with the four Medicaid appointed managed care insurances: UPMC, PA Health & Wellness, AmeriHealth Caritas at Pennsylvania, and Keystone First.
Medicare is a federal health insurance program for people age 65 or older and certain disabled people under 65. It provides care and support services to individuals who have home health care needs for a limited duration. CareSphere has contracts with several Certified Home Care Agencies (CHHAs) who service the clients with home health aides.
Your primary health insurance may be able to cover your home care or home health care needs. CareSphere accepts most of the commercial insurances.
Medicaid is the medical assistance program for low-income people of all ages who are unable to pay for care and who meet the eligibility guidelines. It is financed by federal, state and local funds, and the services covered are set by federal and state law. CareSphere has contracts with Lehigh County and Northhampton County Departments of Social Services for services through the Medicaid Personal Care and Private Duty Nursing Programs.
MANAGED LONG TERM CARE (MLTC)
MLTC provides health and long-term care services to adults with chronic illness or disabilities, to better address their needs and to prevent or delay nursing home placement. Services include – but are not limited to – nursing, physical therapy, occupational therapy, speech pathology, medical equipment and supplies, podiatry, dentistry, optometry, respiration therapy, transportation, and social day care. This program is available to Medicaid recipients who are eligible for admission to nursing homes. CareSphere has contracts with several MLTCs that offer personal care aides.

Veterans of the United States Uniformed Services may be eligible for a broad range of programs and services provided by the Department of Veterans Affairs (VA). Eligibility for most VA benefits is based upon discharge from active military service.
